What You Need to Get Credentialed with Insurance Companies

A practical checklist to gather before you start your first application.

By Melissa Owen Norris

Preparation is the fastest way through credentialing. Have these ready before you apply:

  • Active, unrestricted license
  • NPI 1 (and NPI 2 if you have a business entity)
  • Tax ID / EIN and IRS confirmation letter
  • Current, attested CAQH profile
  • Professional liability insurance certificate
  • Practice address, phone and hours
  • Résumé or CV with no unexplained gaps
  • Diplomas, certifications and DEA if applicable

Common hold-ups

  • CAQH not re-attested every 120 days
  • Names or addresses that don't match across documents
  • Missing work-history explanations
